When the Birth Story Hurts: Recognizing Postpartum PTSD & The Role of a Doula in Recovery

Learn more about the role of a doula in the postpartum period to support postpartum PTSD and other PMADs.

For many, the story of birth is one of joy, strength, and transformation. But for others—even here in Michiana communities like South Bend, Mishawaka, Niles, and Cass County—birth can leave behind something heavier: fear, shame, confusion, or even trauma.

If you find yourself replaying your birth experience over and over again…If you feel panicked, numb, or deeply unsettled weeks or months after giving birth…If you can’t bring yourself to talk about what happened in the delivery room…

You are not alone—and it may be more than just the “baby blues.”

This could be Postpartum PTSD (Post-Traumatic Stress Disorder), and it deserves to be named, understood, and supported. The role of a doula in birth and postpartum support is to avoid birth trauma and help people recovery when it occurs.

What Is Postpartum PTSD?

Postpartum PTSD is a mental health condition that can develop after a traumatic birth experience. It’s estimated that up to 9% of birthing people in the U.S. develop postpartum PTSD, though many go undiagnosed because it’s so misunderstood—or dismissed altogether.

Common causes of trauma during birth include:

  • Emergency interventions or unplanned cesareans

  • Lack of informed consent or feeling ignored by medical staff

  • Birth injuries to baby or birthing person

  • Fear for one’s life or baby’s life

  • Feeling powerless, unheard, or disrespected

  • Previous trauma being reactivated during labor

You don’t need to have had a medically “high-risk” birth to feel traumatized. What matters is how you experienced it.

Signs of Postpartum PTSD

The symptoms can show up shortly after birth—or weeks and months later. They often include:

  • Intrusive flashbacks or nightmares about birth

  • Avoidance of anything related to birth (photos, conversations, etc.)

  • Feeling detached from your baby or yourself

  • Irritability, anger, or emotional numbness

  • Panic attacks or hypervigilance

  • Trouble sleeping or eating

It’s not “just hormones.” It’s not weakness. And it’s not your fault.

The Role of a Doula in Supporting Recovery

As a birth and postpartum doula in the Michiana area, I work with clients who are navigating the complex emotions surrounding their birth story. While doulas don’t replace mental health professionals, our trauma-informed support can be a powerful part of your healing journey.

During postpartum:

  • I provide a safe, nonjudgmental space to process your birth story.

  • I help validate your feelings—especially if others are minimizing them.

  • I offer grounding techniques, feeding support, and emotional care tailored to your experience.

  • I guide partners and loved ones in understanding and supporting your needs.

  • I connect you with mental health professionals who can help further.

During labor (if hired beforehand):

  • I help you advocate for yourself in the birth space.

  • I provide continuous emotional and physical support that centers your experience—not just the medical process.

  • I ensure your voice and preferences are honored, even in unexpected circumstances.
